Download and complete our application packet. This includes our counseling agreement as well as a Personal Data Inventory that will help us become familiar with the basics of your situation before our first appointment.
Submit packet and deposit
Mail or drop off the completed packet along with a $25 refundable deposit made payable to “The Bible Church of Little Rock.” This deposit will be returned when your counseling ends (for any reason) unless you have a last-minute cancellation of a scheduled appointment, in which case it is forfeited.
